新药研发是医学领域研究的重要方向,涉及前期基础研究,动物实验以及临床分析,涵盖领域广,要求知识面宽,综合分析能力强,实践性较高。因此推动学生熟悉相关研究过程,培养一定开发实践能力,对于更好适应未来临床和基础医学、药学相关工作以及相关新疗法和新药物的开发以及医学水平的进步具有重要意义。银屑病是一种常见的难治性的慢性、复发性、炎症性皮肤病。银屑病皮损处特征包括表皮角质形成细胞过度增殖和异常分化,淋巴细胞(绝大多数为T细胞)浸润,以及血管新生、扩张、迂曲、高内皮小血管形成等真皮血管的改变。在银屑病发生和发展阶段,角质形成细胞既是细胞因子作用的重要靶细胞,又是细胞因子产生的重要细胞,也是目前银屑病治疗的重要靶点。银屑病病程长,缠绵难愈,容易复发,大多伴随终生,其带来的瘙痒和疼痛对患者的身心健康及日常生活都带来极大伤害,甚至部分银屑病患者因此抑郁甚至曾有过轻生的想法。银屑病给患者、家庭和整个社会带来巨大的精神和经济负担。该病被世界卫生组织列入世界十大顽症之一,也是皮肤科领域亟待解决的难题。然而由于银屑病发病的复杂性,目前尚缺乏有效的手段对其进行根治。患者须长期服药,但治疗上取得的结果差强人意。许多治疗银屑病的药物存在有严重的毒副作用, 复发率高以及耐药性等问题。中医在银屑病治疗上具有改善病情、复发率低,副作用小,远期疗效好等独特的优势。小檗碱是从中药黄连中分离的一种季铵生物碱,是黄连抗菌的主要有效成分,目前广泛用于临床治疗各种原因引起的腹泻的传统良药,且毒副作用较小。近年研究发现,小檗碱还有许多新的临床用途如降血脂、降血糖等。我们前期研究发现小檗碱可有效抑制角质形成细胞的增殖。提示可能其在银屑病治疗中具有积极作用。本项目拟通过与山东中医药大学及烟台市中医院合作,收集银屑病病例,观察小檗碱外用对银屑病的治疗作用,并进一步利用体外实验分析疗的分子机制。同时项目结合临床疾病背景以及实验室相关技能的课堂知识传授,让学生了解基本的药物研发知识,掌握基本的临床和实验室相关操作技能,熟悉与患者的沟通技巧,更深入的理解临床需求。此外,项目还借助于申请人与烟台绿叶制药有限公司的合作关系,提供学生实地参观药物研发的机会。通过本项目的实施,让学生了解相关临床疾病的基础知识,熟悉药物研发的基本过程,为未来更好学习和适应医学、药学相关岗位的工作奠定良好的前期基础。同时本项目要求参与学生撰写科研报告以及现场答辩,也锻炼了学生的写作能力以及表达交流能力,具有较高的实践应用性。

Psoriasis is a common chronic, recurring, and immune-mediated inflammatory skin disease, with a worldwide incidence of approximately 0.09% to 5.1%. The etiology and pathogenesis of psoriasis are complex and having not been completely elucidated. It is believed that both genetic factors and external stimuli contribute to the development of psoriasis. Current studies show that inflammation, abnormal proliferation and differentiation of keratinocytes, the most predominant cell type of the epidermis, are the key events involved in psoriasis development. A dysregulated crosstalk between epidermal keratinocytes and immune cells leads to epidermal hyperplasia, a hallmark of psoriasis.It is a lifelong disease and the physical discomfort, social stigma and financial burden caused by the disease seriously impairs the life quality of the patients.However, the efficacy of conventional drugs is limited because of adverse side effects and the development of pharmaco-resistance. Therefore, identification of new effective anti-psoriatic agents remains to be an important issue in dermatological research.Natural products isolated from medicinal plants are valuable sources in novel drug development. Berberine (BBR) has been used for treating diarrhoea and gastrointestinal disorders particularly bacterial diarrhea for thousands of years. Clinical investigations on berberine over the years revealed that BBR possesses various pharmacological and biochemical effects, including antihyperglycemic, anticancer, antidepressant, et al. Until now, very little is known about the roles of BBR on keratinocytes. However, the effect and molecular mechanisms of BBR in psoriasis is unknown.Here, we will investigate the effect of BBR in clinical treatment of psoriasis and its molecular mechanisms. Through this project, we could promote the cultivation of clinical skills and medical humanistic feelings of students and offer a good opportunity for they to know the ideas of patients which will be useful for their further jobs.
